Patient Rights

As a patient served by the Annette Island Service Unit, you have:

  1. The right to be treated with respect, consideration, and dignity.
  2. The right to receive preventive and curative services that are based on current standards of care and knowledge.
  3. The right to receive these services in a manner that is responsive to your cultural, emotional, and spiritual needs.
  4. The right to participate in decisions involving your health care.
  5. The right to a full explanation and understanding of the diagnosis and of the nature and purpose of all proposed treatments, prescribed drugs, or diagnostic procedures. This includes the right to fully understand the risks, side effects, and probable outcomes of treatment, the reasonable treatment alternatives, and the consequences of no treatment.
  6. The right to consent to or to refuse any procedure, test, or treatment (to the extent permitted by law) and to be informed of the probable consequences of this action. The Annette Island Service Unit will assist patients who request help preparing Advance Directives and Living Wills.
  7. The right to consult with a health care provider about emergency conditions and to receive treatment in a timely manner.
  8. The right to know the name, professional training, and professional licensing of personnel involved in your care and treatment.
  9. The right to choose a health care provider for appointments at the Outpatient Medical Clinic.
  10. The right to request and receive a second opinion in cases of major surgery or if you are not responding to treatment.
  11. The right to review records pertaining to your care in the presence of a health care provider.
  12. The right to privacy and confidentiality for all services, information, and records, except when released by your written consent or by court order or judicial requirement. Federal guidelines for the confidentiality of patient records related to drug and alcohol abuse will be followed.
